Sinopsis

The new edition includes increased video/DVD coverage, new chapters on BRAVA tissue expansion, flaps, and increased detail on mega-filling the breast with fat. The book is published with a bundled eBook, which includes an image library so all the images can be downloaded.

»Partial Breast Reconstruction« is a groundbreaking work in reconstructive surgery, edited and written by international leaders in this field. It presents the latest reconstructive techniques for breast cancer patients who have breast conserving therapy (BCT), where theprimary treatment is partial mastectomy and radiation therapy.

With the "oncoplastic approach" women are now able to preserve their breasts while maintaining a breast that is aesthetic, symmetric, and well shaped. Today, BCT has become the standard of care for many women diagnosed with breast cancer. Unfortunately, theincidence of poor cosmetic results is relatively high with standard BCT.

This book provides surgeons with the techniques needed to improve aesthetic outcomes while maintaining oncologic standards. It focuses on three critical areas: 1) improving the breast resection to ensure the most aesthetic outcome, 2) preventing a deformity withpartial breast reconstruction when a poor cosmetic outcome is anticipated, and 3) correcting a post-radiation deformity when it occurs. A Timely Topic The oncoplastic approach is the wave of the future.

It is imperative for reconstructive surgeons to learn oncoplastic techniques so that they can broaden their practices and take care of this growing population of patients with BCT deformities. This comprehensive book will provide you with the clinical information andguidance that you need to incorporate this approach into your practice. Total Coverage This beautifully illustrated semi-atlas provides complete coverage of partial breast reconstruction in 31 comprehensive chapters divided into 5 parts. Part I includes key chapters on oncoplastic surgery basics and incorporating oncoplastic surgery into your practice. Part II includes chapters on applied anatomy, decision making, improving the partial mastectomy and quadrantectomy defects, and radiotherapy after oncoplastic surgery. Focusing on immediate partial breast reconstruction and correction of the BCT deformity with delayed partial breast reconstruction, Parts III and IV are the largest sections of the book. Chapters in these sections provide step-by-step details for the range of oncoplastic techniques.

Hot Topics:

Treatment algorithm for preventing the BCT deformity
Partial breast reconstruction using parenchymal rearrangement, reduction techniques, and local breast flaps
Partial breast reconstruction using pedicled perforator flaps
Fat grafting for correcting post-BCT deformities
Management of central defects
Parenchymal autoaugmentation techniques
Endoscopic reconstruction of the partial mastectomy defect

Editorial Reviews

Reviewer: Jeffrey S. Rosenthal, MD (Bridgeport Hospital)
Description: The ubiquitous nature of breast cancer requires a multidisciplinary approach to the extirpation and reconstruction of the breast. Modern breast conserving techniques are described along with the oncologic evaluation of the breast. This book offers valuablealternatives to the standard reconstructions of the past and will be a benchmark by which breast surgeons should measure their end results.
Purpose: The authors wish to provide surgeons with substantive techniques that will allow for removal of breast tumors using breast conservation methods and the subsequent reconstruction of the breast, affording the patient optimal aesthetic results. They wish to "improve the standards of care for women with breast cancer." In a world where breast cancer does not discriminate, any improvement which will allow for removal of the tumor, preservation of breast tissue, and the reshaping of the remaining breast to a relatively normal form is both laudable and appreciated.
Audience: The authors are experienced oncologic and reconstructive physicians who have a deep interest in the salvage of the breast shape and proportion as well as the treatment of breast cancer. Oncologic and plastic surgeons, along with those recommendingadjuvant therapy (chemotherapy and radiation) should absorb this book before undertaking their respective roles in either the removal of breast tumors or the restoration of the breast. It is not a primer for beginners, but it can be used to start the process by those muchless knowledgeable.
Features: Breast conservation therapy crosses many specialties and, as such, necessitates an understanding of the various topics that will impact upon breast care. The authors have focused on breast reconstruction techniques for rebuilding partial breast defects, buthave wisely included a plethora of information regarding the evaluation, indications for breast conservation therapy, psychological aspects of patient care, radiotherapy challenges, monitoring for recurrence of breast cancer, and other variables that must be considered to obtain both maximal patient safety and aesthetic outcomes. The book is divided into five parts, commencing with the evolution of oncologic surgery in North America and abroad. Principles of breast conservation are addressed next, followed by techniques for immediatepartial breast reconstruction. The fourth part delves into deformities encountered with breast conservation followed by a delayed reconstruction. Concluding thoughts are concerned with future directions of care and the outcomes expected from partial reconstructionfollowing breast conservation. Superb photographs, drawings, and didactic charts adorn this well crafted book. The authors highlight essential caveats that will assist in a good patient outcome. Accompanying the book is a DVD that takes users into the operative theatre to watch various partial breast reconstructive surgeries.
Assessment: The authors and contributors are to be commended for assembling an important book on a subject that is just coming to the forefront of breast reconstruction therapy. Their techniques and valuable teachings will benefit those who desire optimal outcomesfor their patients.
